Health Office Back To School Forms

Please provide your student’s annual physical assessment to the health office. Annual physical examinations are valid for 13 months.
  • BMI Screening: Grades 1, 4, 7 & 10
  • Postural Screening: Grades 5-9
  • Vision Screening: Grades 1-5, 6-8, 10
  • Hearing Screening: Grades K-3, 6-8, 10
  • SBIRT: Grades 7 & 9
All updated immunizations should be on file with the health office. We are requesting all immunization submitted by the start of the school year.

Massachusetts now is requiring Influenzas vaccinations for all students.
Students grades 7 through 10 are required to receive one dose of the Meningococcal vaccination and grades 11 and 12 two doses of the meningococcal vaccination.

Remote learners are not exempt from vaccination.
If you require an exemption, please send the health office a signed letter of exemption.

During the school year, any student that requires medication administration will be required to fill the forms below.


Asthmatic students are urged to bring spacers with their metered-dose inhalers to decrease the transmission of aerosols.


Individual Collaborative Health Plan (ICHP) is required for students with more complex health needs and services. Please contact your healthcare provider to submit an Individual Collaborative Health Plan or an Action Plan to the health office for your student.

Snowden is one of the few schools in the district that has the opportunity to participate in a school-based health center with a partnership with the Boston Health Commission and Boston Medical Hospital. The school-based health center is available on Tuesdays and Thursdays for a scheduled appointment with nurse practitioner Elisabeth Blair for screenings, physicals, etc. If you would like your student to participate, please fill out the form below and return it to the health office.

Everyone is expected to wear a mask whenever inside a BPS building or vehicle, or outside on school grounds when others are within six feet or less.


 Accommodations will be made for our youngest learners, students with sensory issues, and for our most vulnerable and medically fragile special needs students, as appropriate and necessary.

BPS expects all students to bring and wear their own cloth masks whenever possible and appropriate.


Anyone who forgets their reusable mask will be provided a disposable mask to wear on the bus and within the school building.


Face masks must cover the mouth and nose. Face masks must be washed daily.


Students are prohibited from wearing the following mask: neck gaiters, open chin triangle bandanas & those that contain mesh material, valves, or holes of any kind.


Unacceptable Face Mask

Families and students will be asked to monitor themselves daily for entering the school building. Students will be asked to complete the Home Health Checklist at home (please do not submit to the health office).


If the student or anyone in the home has any of the symptoms of COVID-19 please stay at home and inform the school nurse.


If a student has a chronic condition that mimics COVID-19 symptoms, such as seasonal allergies, please submit a note from the student’s primary care provider.
